Assumption of Risk *

I acknowledge that participation in golf simulation, fitness activities, and any use of the Facility involves certain inherent risks, including but not limited to the risk of injury from swinging golf clubs, errant shots, slips, trips, falls, or equipment malfunction. I voluntarily assume all such risks, both known and unknown.

Release of Liability *

In consideration of being granted access to the Facility, I hereby release, discharge, and hold harmless Phoenix Golf Lounge, Phoenix Wellness Center, LLC, its owners, officers, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage that I may sustain while in or around the Facility, whether caused by negligence or otherwise.

Indemnification *

I agree to indemnify and hold harmless Phoenix Golf Lounge and its affiliates from any loss, liability, damage, or cost, including court costs and attorney’s fees, that may arise due to my presence in or use of the Facility.

Health & Conduct *

I certify that I am physically able to participate and understand that I am responsible for my own actions and the safety of others while using the Facility. I agree to use the Facility responsibly and to comply with all posted rules and staff instructions.

Minors *

If signing on behalf of a minor, I affirm that I am the parent or legal guardian of the participant and accept full responsibility for the minor’s use of the Facility.

Acknowledgment *

I have read and voluntarily agree to this Liability Waiver & Release. I understand that by accessing the Facility and receiving the entrance code, I am waiving certain legal rights.
